Özet

This prospective, double blind study investigates the hemodynamic and neuromuscular effects of atracurium, mivacurium and the combination of them. 45 ASA I-II class patients undergoing hydroselectomy or varicoselectomy were studied. After fentanyl 1.5 μg/kg and propofol 2.5 mg/kg for the induction, group I (G1; n=15) received atracurium 0.5 mg/kg, group II (G2; n=15) mivacurium 0.25 mg/kg and group III (G3; n=15) atracurium 0.25 mg/kg + mivacurium 0.125 mg/kg. Heart rate (HR) and mean arterial pressure (MAP) were recorded just before the induction (T0) and at each of the following 5 minutes. The time to T4/T1 ratio depression to 20 % (the onset time) and T4/T1>0.25 (the recovery time) were measured. The onset time in G1, G2 and G3 were 233±64.8 sec, 200±79.4 , 210±45.7 sec respectively (statistically not significant); and the recovery time were 27.7±5.4; 16.8±2.9; and 21.5±3.4, respectively (p<0.05). The MAP levels were not significantly different in recording times but the decreasing rate of MAP were more significant in G2 and G3 than G1. (G1: p=0.025; G2 and G3: p<0.0001). The use of these muscle relaxants in combination has the advantage of better manipulating the duration time of the neuromuscular blockade. But it must be emphasised that mivacurium, used in intubating doses or in combination of mivacurium with atracurium can reduce the MAP more than atracurium alone.
